Transmission shifts rough on 2000 Toyota Sienna

Q.What makes my Sienna transmission shift so hard in the 1st 2 gears?
I already checked/done: Fluid level good, and fluid clean

Before transmission could be diagnosed more information needed:
Where do you drive mostly your Toyota?
How many miles on it?
Did you do transmission oil change in the past? How long ago?
Did it start shifting rough out of the blue or there was something you that caught your attention?
The reason I asked those questions is because if car used mostly in the city - it makes much more shifts that in suburbs and on average it has a higher temperature which leads to more extensive wear and tear of the clutch disks.
