How to replace a serpentine belt tensioner on 96' Ford Taurus?

Q.My serpentine belt gets shredded every week by misaligned belt tensioner.
Anybody knows how to replace the tensioner?
I already checked/done: Checked Google - no straight answer..
Answer I had the same issue.

As you can see in a picture above - serpentine belt tensioner was misaligned due to the fact that when it was replaced mechanic forgot to put a washer under the holding bolt.

To remove the tensioner you'd need a T50 torx wrench to remove the bolt that holds it and an idler pulley.
To remove the belt itself just use a 15mm wrench and the other wrench as an extension and rotate it clockwise (when looking at the engine from the left side of the car).
